CED Player Belt Ordering Form for Winter 2008

 

This page will produce a printable form you can mail with payment via the postal service. If you want to purchase belts via credit card, go to the Products Page and add them to your shopping cart. If you are uncertain about the proper name or location of a belt, first consult the Belt Guide. To use this order form follow these steps:
  1. Fill in the address to mail the belts to at the top of the form.
  2. Select up to ten belts by clicking on the selection boxes and pointing to the desired belt. Choose only one belt per box. For example, if you need a quantity of three of the same belt, select that belt in the Belt No. 1, Belt No. 2, and Belt No. 3 boxes. If more than ten total belts are required, print out a second copy of the form.
  3. Print the page. If a printer is unavailable, write down all the information below, including the full name of the belt as listed in the selection boxes.
  4. Add up the dollar amounts listed next to each belt on the form. This is the total amount to send. Please send payment in the form of cash, as my bank gives me a hassle about cashing $1.00 checks.
  5. Mail the printed form with payment to this address:

  6. Allow three weeks for delivery of the belts via the U.S. Postal Service.

Note on Elmo, Toshiba, and Wards Turntable Drive Belt:
This turntable drive belt is not an exact size replacement-- the belt is shorter and narrower than the OEM belt, but it is the closest equivalent available. The belt has been tested and found to work, but will periodically fall off the unflanged drive pulley during disc load or unload due to the narrower width. I recommend the $2 belt only to test the player for proper operation prior to implementing the $30 permanent solution.
